Photo ID
Payment – we accept cash, personal check, debit cards, American Express, Discover, MasterCard and Visa and provide you with a receipt you can file with your insurance company
A list of all your current medicines with dosages
A list of any drug allergies
Medicare or Medicare Advantage card
Tricare Standard ID card
We have provided our most frequently used forms and policies online for your convenience. Please feel free to print and complete any forms you may need prior to your visit; otherwise we can provide them to you upon arrival to the office.
